Adult polycystic kidney disease is often associated with cystic liver disease, but diaphragmatic herniation and compression of the right atrium by a hepatic cyst is a previously unreported finding. This case describes a patient with adult polycystic kidney disease and chronic renal failure who presented with chest pain, and was initially suspected to have had uremic pericarditis. Echocardiography revealed this surprising finding. The patient improved with symptomatic treatment. A review of adult polycystic kidney disease and polycystic liver disease is presented.
