A 47-year-male patient with ischemic cardiomyopathy underwent electrophysiological study and catheter ablation for recurrent ventricular tachycardia (VT). Despite a recent history of several episodes of VT, no arrhythmia could be induced by programmed stimulation; therefore, substrate mapping was conducted. Endocardial mapping revealed no favorable site. The epicardium was accessed according to the Sosa technique with a Tuohy peridural needle (Perican, Melsungen, Germany), under fluoroscopic guidance in lateral projection
