This report describes the case of a 22‐year‐old patient presenting with ICD electrode failure where attempted percutaneous electrode extraction was unsuccessful. In view of the patient's young age, an operative approach was considered most appropriate. The failed electrode was surgically extracted and a new epicardial pacing/defibrillator system inserted.
