Discordant ST elevation >5 mm was recently described as an electrocardiographic sign with good specificity and moderate sensitivity for the diagnosis of acute myocardial infarction during right ventricular pacing. This report shows that the same sign may occur transiently during myocardial ischemia and also serve as a diagnostic marker.
