We present the case of a 23‐year‐oid patient who underwent orthotopic heart implantation and subsequent implantation of an atrial dual chamber pacemaker. The stimulation rate in the donor atrium is controlled by the recipient's sinus node activity, thereby maintaining AV synchrony and allowing appropriate rate increase during exercise.
