An AV universal (DDD) pacemaker‐mediated, reentrant, endless loop tachycardia initiated by a reciprocal beat is described. Pacemaker‐mediated tachycardias are discussed and addressed in the light of their growing frequency and importance in patients with physiological pacemakers.
