Open heart operations are sometimes performed without donor blood because of religious beliefs of the patient. A medical reason for performing surgery without donor blood occurs when the patient has an antibody to which no available donor blood is compatible. In the case reported, two units of blood were removed from the patient during a 48‐hour period prior to an aortic valve replacement. These were transfused to him postoperatively with good results.
