SUMMARY Three cases of tearing of the dorsal aspect of the diaphragm are described. Comparisons have been drawn between dorsal diaphragmatic hernias. In all three cases the right dorsal aspect of the diaphragm was involved in the tear. Two cases also had the right diaphragmatic crus torn. The paracostal abdominal approach is recommended to enable adequate visualization and repair of the dorsal aspect of the diaphragm.
