The medical director's role in the nursing home will continue to be a crucial one, but because of the limited availability of physicians, this role should be redefined realistically to insure economical and appropriate use of physicians' services. The roles of administrator and of director of nursing services also need redefinition for effective use of their disciplines and to avoid confusion in their functions. The nurse's role should be expanded because of the needs of the patient and the availability and competency of nurses to care for the aged and chronically ill. This would result in better use of the services of both physicians and nurses. The members of all three disciplines should collaborate in redesigning the nursing home programs to meet the intense and complex needs of the patients.
