A case of splenomegaly probably secondary to malaria in utero is presented. The patient was a 50‐year‐old male who was admitted to the hospital because of longstanding abdominal pain and swelling. Otherwise he had had no serious illnesses and no symptoms of malaria. Splenectomy was performed because of the hazard of injury to the organ in the patient's occupation as a laborer handling heavy machinery.
