Revascularization of Severely Ischemic Lower Extremities in the Elderly by Femorotibial Bypass *

In 79 of 89 patients operated on for severely ischemic lower extremities, the procedure used was femorotibial or femoroperoneal bypass. Two thirds of the patients were over 60 years of age. In some cases the follow‐up period was as long as eight and a half years. A functional bypass was achieved as often in the older groups (67–77 per cent) as in the younger group (74 per cent). The rate of limb salvage was lower in the older groups, but it was still a significant 57–65 per cent versus 74 per cent in patients younger than 60. Femorotibial bypass in lieu of primary amputation is recommended for elderly patients with such absolute indications for surgery as gangrene, gangrenous ulceration, or rest pain.
