A bstract The author outlines his experiences with milieu therapy in the establishment of a therapeutic community in a geriatric day hospital. Both the benefits and the difficulties are discussed. The findings eventually led to an “agonizing re‐appraisal” of methods, in order to avoid filling the day hospital with chronically ill and excessively dependent (but complaining) patients rather than with those who have acute disorders and need more immediate rehabilitation. Despite setbacks, the effort to create an optimal therapeutic milieu seems well worth while.
