A summary is given of the latest advances in our knowledge of the fundamental processes in diabetes, including the nature of insulin and the regulation of its production, the role of glucagon in diabetes, and the problem of the associated vascular disorders. Intensified research is needed in the areas of more specific criteria for diagnosis, etiology and pathogenesis, and the precise mode of insulin action at the molecular level; also, the basis for the acceleration of arteriosclerosis in diabetes, and the possible relationship between microangiopathy and the abnormal nutritional metabolism. Successful implantation of functioning human islet tissue is another goal.
