Summary:  We report a 47‐year‐old right‐handed woman with simple partial seizures manifesting in part as an urge to defecate shortly after a visual distortion. On at least one occasion, this was followed by head deviation to the left and a secondarily generalized tonic–clonic seizure. The ictal electroencephalogram lateralized to the right hemisphere, suggesting that the ictal urge to defecate may lateralize to this side.
