SYNOPSIS Transfemoral carotid angiography was performed in a 23‐year‐old man twelve hours after the abrupt onset of hemiplegic migraine. An unusual appearance of the internal carotid artery contralateral to the hemiplegia was noted. Possibly this appearance represented spasm.
