SYNOPSIS A patient is reported who has had 4 bouts of typical left‐sided cluster headache, each lasting for about 6 weeks. In the last 3 of these he developed a right lateral rectus palsy, which persisted unchanged throughout each cluster and settled about a week after the pain subsided. No alternative cause for the palsy could be identified and its mechanism remains obscure, though it may be in some way related to ophthalmoplegic migraine.
