SYNOPSIS Post spinal puncture headache has been a bothersome problem ever since the introduction of the spinal tap over a century ago. It is generally conceded to be due to leakage of spinal fluid through the hole produced by the needle. In this study in‐vivo and in‐vitro evidence suggests that tangential puncture of the subarachnoid space leads to less leakage than perpendicular puncture and that the size of the needle used may not be as important as the angle from which it pierces the dura and arachnoid membranes.
