SYNOPSIS A patient with severe bilateral carotid occlusive disease and paroxysmal hypertensive attacks associated with vascular headaches localized to an area of a major collateral system is reported. The similarity of the patient's symptoms to many components of migraine illustrate the relationship of headache, conjunctival erythema and unilateral lacrimation to increased blood flow.
