  We first report a case of protein losing enteropathy in severe atopic dermatitis in an exclusively breast‐fed 5‐month‐old infant. Protein losing enteropathy was confirmed by fecal α 1 ‐antitrypsin clearance test and imaged successfully by 99m Tc‐human serum albumin scintigraphy. The present case highlights that protein losing enteropathy in severe infantile atopic dermatitis is being a topic of concern and also an issue even in exclusive breast feeding patients.
