Where do the new oral biphosphonates (like alendronate) fit in to the management of renal osteodystropy? I realize the official recommendation is not to use alendronate in advanced renal failure. However, I believe that this is based solely on a lack of data and that some level of dosage reduction for this unmetabolized renally excreted drug should be all that's needed.
