Summary An appreciation of the natural history of SLE following the development of ESRD is important in the management of these patients. In addition, the care of these patients requires special consideration in the diagnosis and treatment of disease exacerbations. It is ironic that, despite the destruction of a major organ system as a result of a disease process, the clinical course of that disease may improve. Such appears to be the case with SLE. The prognosis for renal transplantation is even more encouraging. Patients can therefore enter an ESRD program with optimism.
