This case report accounts the history of a 71‐year‐old man who presented with a 1‐month history of cognitive decline, the diagnosis of which was only made at post‐mortem, and was found to be paraneoplastic limbic encephalitis secondary to an unsuspected non‐small cell lung cancer. The epidemiology, clinical and pathological features of paraneoplastic limbic encephalitis are then discussed, as are potential mechanisms by which the condition may be produced.
