This contribution was submitted for consideration for publication as a letter to the editor. The author is one of Australia's most senior obstetricians. The editor has known him well for 45 years and considers him to be both an impressive scholar and formidable iconoclast. In addition he is writing on his favourite subject, concerning which he has analyzed the relevant literature for many years, often in relation to providing opinions in cases being considered for litigation. He also has the distinction of usually providing opinions from the point of view of patients rather than defendants (obstetricians, hospitals) in such cases. We have not invited the authors whose papers are mentioned to respond to his comments. The editorial committee of the journal and our other reviewers agreed that this material should be offered to our readers as it stands for their consideration.
