EDITORIAL COMMENT: We accepted this case for publication because it warns us again that postoperative distension can be associated with spontaneous perforation of the colon. The authors provide a detailed account of the appropriate management. Readers are referred to the previous editorial comment on Ogilvie syndrome ‘Atypical Ogilvie syndrome’. Paraskevaides E C. To update the comment the case reported here is the first to occur in 112,280 confinements including 16,548 Caesarean sections at the Mercy Hospital for Women, Melbourne to August 31, 1994.
