EDITORIAL COMMENT: We accepted this case report for publication to inform readers that pyometra is an entity in itself with a significance apart from that of underlying pathology such as carcinoma of the cervix or endometrium. The authors' takeaway message seems to be that a pyometra needs treatment by cervical dilatation in addition to that of associated complications including peritonitis and septicaemia as a result of perforation of the uterus.Summary: Spontaneous uterine perforation of pyometra is closely related to physiological uterine involution and requires a correct diagnosis, rapid surgical intervention and administration of antibiotics.
