Summary: A patient presenting with pre‐eclampsia in her second pregnancy was found to have persistently low urinary oestriol levels (below 2mg/24 hours) in the last trimester. Plasma progesterone estimation showed that this reading was not due to placental insufficiency, and radiological examination excluded anen‐cephaly. No drugs known to affect the measurement of urinary oestriol were being ingested. A normal male child was delivered by Caesarean section. Subsequent studies confirmed the presence of a placental sulphatase deficiency.
