Summary: The surgical management of long‐standing obstetric recto‐vaginal injuries is discussed. Rational treatment is based on present knowledge of anatomy and physiology of ano‐rectal continence. Emphasis is laid on the repair of the superficial portion of the external sphincter, and the relative unimportance of the subcutaneous sphincter in anal continence is noted. Two cases are reported, and attention is directed to a simple scheme of pre‐operative and postoperative management.
