Summary The physiology of micturition and the mechanisms involved in maintaining continence are briefly outlined. The place of a combined synchronous vaginal and suprapubic operation in the management of stress incontinence is discussed and one variant of such an operation is described. The importance of complete vaginal and suprapubic dissection of the urethra and bladder neck is stressed, particularly in patients with recurrence, before proceeding to surgical correction. It is in such patients that the synchronous operation is particularly recommended.
