Summary The literature on arrhenoblastoma is briefly reviewed and a large undifferentiated tumour resulting in extreme virilisation is reported. Personality changes associated with progressive defeminisation and their disappearance on extirpation of the tumour are discussed. Conservative surgical treatment appears to have been justified as the follow‐up period approaches 5 years.
