Summary High rectovaginal fistulae following obstructed labour may be impossible to repair by a vaginal operation. An abdominal operation is described in which the rectum is mobilized to the pelvic floor, separated from the vagina, repaired and a pedicle graft of greater omentum interposed between the rectum and vagina. This operation has been successful in five consecutive patients.
