Summary A patient with sickle‐cell anaemia whose twin pregnancy was complicated by pre‐eclampsia and repeated sickling crises is described. Partial exchange transfusion of the mother at 34 weeks gestation achieved a marked reduction of haemoglobin‐S. After premature onset of labour, live twins were delivered by Caesarean section within 24 hours of exchange transfusion. Some aspects of management are discussed, and the literature is briefly reviewed.
