Summary The clinical findings and management of 39 patients with large postpartum broad ligament haematomas are described. Eighteen cases occurred immediately after delivery, lower abdominal pain, collapse and signs of internal haemorrhage being constant features. In 21 patients the haematoma formation was delayed, and in these anaemia, pyrexia and secondary postpartum heamorrhage were common modes of presentation. Twenty‐six patients were treated by blood transfusion and chemotherapy, 5 had the uterus and vagina packed, and 8 patients were treated by abdominal hysterectomy. There was one maternal death. A review of the literature and divergent views about the management is presented.
