Myelopathy due to Atlanto‐axial Dislocation in a Patient with Down's Syndrome and Rheumatoid Arthritis

Summary A case of Down's syndrome in association with rheumatoid arthritis is described. The patient had spastic diplegia and atlanto‐axial dislocation, probably the result of ligamentous laxity which is common in both Down's syndrome and rheumatoid arthritis. This laxity may have been enhanced by tonsillar or deep cervical infection. The myelopathy was thought to be due to the atlanto‐axial dislocation. Spinal fusion may reverse the neurological abnormality in some cases. RÉSUMÉ Myelopathic à cause de luxation atlanto‐axiale dans un cas de Down et reumatisme articulaire Un cas de syndrome de Down en association avec rheumatisme articulaire est exprimé. Le malade avais diplegie spastique et fixation atlanto‐axiale probablement a cause de laxité ligamenteuse qui sont commun en mongolisme et rheumatisme articulaire. La laxité ligamenteuse peut être augmenté par amygdale, et/où de profonde infection cervicale. La myelopathic était a cause de luxation d'atlanto‐axiah La fusion vertebrate peut renverse Pabnormalite neurologique dans quelques cas. ZUSAMMENFASSUNG Ein Kranken Bericht von Myelopathy in einen Patienten schuldig durch Atlanto‐axial Verrenkung mil Down Syndrom und Reumatoid Arthritis In diesem Artickel wurde ein Fall von Down Syndrom in Verbindung mit Rheumatoid Arthritis beschrieben. Der Patient hatte Spastic Diplegia und Atlanto‐axial Verrtnkung wahrscheinlich auf Grund von losen Ligamenten, gemein in Beiden Mongolisum und Rheumatischen Arthritis. Die losen Ligaments könnten erhoht werden von Mandeln und/oder tiefen Nacken Infektion. Die Myelopathy war Schuld an der Atlanto‐axial Verrenkung. In manchen Fallen eine Ruckgrat Fusion könnte die Nerven Regelwidrigkeit unwenden. RESUMEN Presentacion de un caso de mielopatia debida a dislocacion atlanto‐axial en un paciente con sindrome de Down y artritis reumatoidea Se describe un caso de sindrome de Down asociado con artritis reumatoidea. El paciente presentó diplegia espástica y dislocación atlanto‐axial, debida probablemente a laxitud de los ligamentos, lo que es comtin tanto a mongolismo como a artritis reumatoide. La laxitud de los ligamentos puede haberse hecho aparente por la infección de las amigdalas y/o la infeccion cervical profunda. La mielopatia se debio a la dislocatión atlanto‐axial. La fijación de la columna puede mejorar las anormalidades neurologicas en algunos casos.
