Post‐operative Neuropathic Fractures in Patients with Myelomeningocele

Summary Neuropathic fractures are common after major surgical procedures to stabilize the hip or spine in patients with myelomeningocele. Complications resulting from such fractures are frequently observed. The fractures occur particularly in the more severly involved children and are associated with prolonged immobilization after surgery. The authors advise that all necessary surgery be done within the shortest possible time, and urge that prolonged immobilization be avoided if possible. RÉSUMÉ Fractures neuropathiques post‐opéraloires dans le myéloméningocèle Les fractures neuropathiques sont fréquentes à la suite d'interventions chirurgicales majeures pour stabiliser la hanche ou la colonne vertébrale chez des sujets présentant un myéloméningocéle. On observe fréquemment des complications à la suite de telles fractures. Les fractures surviennent tout particulièrement chez les enfants les plus atteints et sont associées avec la prescription d'une immobilisation prolongée aprés la chirurgie. Les auteurs indiquent que toute la chirurgie nécessaire devrait être faite dans les délais les plus courts et demandent que l'immobilisation prolongée soit autant évitée que possible. ZUSAMMENFASSUNG Postoperative neuropathische Frakturen bei Patienten mil Myelomeningocele Nach großen chirurgischen Eingriffen zur Stabilisieurung der Hüfte oder der Wirbelsäule kommt es bei Patienten mit einer Myelomeningocele häufig zu neuropathischen Frakturen. Es wurden häufie durch solche Frakturen entstandene Komplikationen beobachtet. Die Frakturen trétén besonders bei den schwer betroffenen Kindern auf und stehen im Zusammenhang mit langen Immobilisationsperioden nach der Operation. Die Autoren geben den Rat, die notwendigen operativen Eingriffe im möglichst kurzer Zeit zu erledigen und warnen eindringlich vor langeren Immobilisationsperioden. RESUMEN Fracturas neuropáticas post‐operatorieas en el mielomeningocele Las fracturas neuropáticas son corrientes después de practicar una cirugia mayor para estabilizar la cadera o la columna vertebral en pacientes con mielomeningocele. Las complicaciones resultantes de estas fracturas se observan con frecuencia. Las fracturas tienen lugar especialmente en los niños más gravemente afectados y están asociados con una larga inmovilización después del acto quirúrgico. Los autores aconse jan que todo acto quiriirgico necesario debe ser hecho en un tiempo lo más corto possible y piden que la inmovilización prolongada sea evitada dentro de lo posible.
