Transient Thrombocytopenia in a Child on Sodium Valproate

SUMMARY Thrombocytopenia occasionally has been reported in patients on sodium valproate. In the present report a seven‐year‐old girl on sodium valproate presented with bronchopneumonia and was found to be thrombocytopenic. Because of the efficacy of the drug in reducing the number of seizures in this child, the anticonvulsant was continued. The pneumonia resolved with ampicillin treatment and the platelet count returned to normal. Three months later, and while the child was still on sodium valproate, a second transient fall in the platelet count occurred, but soon recovered. It is suggested that platelets should be monitored in children with respiratory infections who are being treated with sodium valproate but that the presence of thrombocytopenia need not necessitate discontinuation of the drug. RÉSUMÉ Thrombocytopénie transitoire chez un enfant traité par le valproate de sodium Des thrombocytopénies ont été rapportees occasionnellement chez des malades traités par le valproate de sodium. Cet article rapporte le cas d'une fillette de sept ans traitée au valproate de sodium, ayant présenté une broncho‐pneumonie avec découverte à cette occasion d'une thrombocytopénie. Le traitement anticomitial réduisant le nombre de crises fut continué. L'état pneumonique se résorba sous traitement d'ampicilline et le taux de plaquettes revint à la normale. Trois mois plus tard, l'enfant étant toujours sous valproate de sodium une seconde chute transitoire du taux des plaquettes fut observée avec une évolution rapidement favorable. L'auteur suggère que les plaquettes doivent être surveillées chez les enfants sous valproate de sodium présentant des infections respiratoires mais que la présence d'une thrombocytopénie ne nécessite pas l'interruption du valproate. ZUSAMMENFASSUNG Vorübergehende Thrombocytopenie bei einem Kind, das mit Sodium Valproate behandelt wurde Es ist gelegentlich über eine Thrombocytopenie bei Patienten berichtet worden, die mit Sodium Valproate behandelt wurden. Die vorliegende Studie berichtet über ein 7 jähriges Mädchen mit Sodium Valproate Therapie, das wegen einer Bronchopneumonie untersucht und bei dem eine Thrombocytopenie gefunden wurde. Auf Grund der Wirksamkeit des Medikamentes zur Senkung der Anfallshäufigkeit wurde bei diesem Kind das Antikonvulisum weitergegeben. Die Pneumonie wurde mit Ampicillin beherrscht und die Thrombocytenzahl normalisierte sich. Drei Monate spärer, nachdem das Kind weiterhin mit Sodium Valproate behandelt worden war, erfolgte der zweite vorübergehende Thrombocytenabfall, jedoch normalisierte sich die Thrombocytenzahl sehr rasch. Es wird empfohlen, dass bei Kindern unter Sodium Valproate Therapie die Thrombocytenzahl bei respiratorischen Infekten kontrolliert werden sollte, eine Thrombocytopenie jedoch nicht das Absetzen des Medikamentes erfordert. RESUMEN Trombocitopenia transitoria en un niño con valproato sódico La trombocitopenia ha sido descrita ocasionalmente en pacientes con valproato sódico. En la presente comunicación se describe una niña de siete años que tomaba valproato sódico y que presentó una bronconeumonía hallandosele una trombocitopenia. Debido a la eficacia del fármaco para reducir el número de convulsiones, en este niño se continuó el anticonvulsionante. La neumonía se solucionó con ampicilina y el contaje de plaquetas se normalizó. Tres meses más tarde, y mientras el niño tomaba todavía valproato sódico, tuvo lugar una segunda caída transitoria de plaquetas pero se recuperó pronto. Se sugiere que las plaquetas deberían ser monitorizadas en niños con infecciones respiratorias y que son tratadas con valproato sódico, si bien la presencia de una trombocitopenia no debe necesariamente interrumpir el fármaco.
