Nervous Endemic Cretinism in Eastern Nepal

SUMMARY 31 children and adolescents from the mountainous region of Eastern Nepal were examined in order to outline the clinical pattern of cretinism found there. They correspond to the description of nervous endemic cretinism found elsewhere: psychomotor retardation, deafness, and a variable degree of spastic diplegia. There was no clinical evidence of hypothyroidism. The disorder appears to be related to gestational iodine deficiency and it is suggested that injections of iodinated poppyseed oil (‘Lipiodol’) would be the most suitable form of prophylaxis. ZUSAMMENFASSUNG Neurologisch endemischer Kretinismus in Ostnepal Es wurden 31 Kinder aus der Gebirgsregion von Ostnepal untersucht, um das klinische Bild des dort auftretenden Kretinismus zu beschreiben. Die Kinder zeigten die Symptome eines neurologischen, endemischen Kretinismus, wie er andernorts beschrieben wurde: psychomotorische Retardierung, Taubheit und spastische Diplegie mit wechselnder Ausprägung. Es fand sich kein klinischer Hinweis für eine Hypothyreose. Das Krankheitsbild beruht wahrscheinlich auf einem Jodmangel während der Schwangerschaft. Es wird deshalb angeregt, jodhaltiges Mohnsamenöl (‘Lipiodol’) als prophylaktische Maßnahme zu injizieren. Los niños respondían a la descriptión del cretinismo nervioso endémico encontrado en otros lugares: retraso psicomotor, sordera y un grado variable de diplegia espástica. No había evidencia clínica de hipotiroidismo. La alteractión aparece en relación con la deficiencia de yodina gestacional y se sugiere que la forma más adecuada de profilaxis sería inyecciones de aceite yodinado de semillas de amapola (‘Lipiodol’).
