Perceptuo‐motor Dysfunction in the Child with Hemiplegia

SUMMARY Eighteen children with hemiplegia were examined by means of standard neurological and other, specialized, sub‐tests to define the extent of their perceptuo‐motor dysfunction. Particular attention was paid to auditory‐language function, praxis, visuo‐spatial skills, body schema, tactile defensiveness and inattention. The side of the hemiplegia was not found to predispose to any specific dysfunction, with the exception of inattention which was seen only in those with left‐sided hemiplegia. Children with a history of familial sinistrality had a higher incidence of speech problems, particularly in the area of auditory language and speech delay. The results are discussed in the light of therories of plasticity and cerebral organization. RÉSUMÉ Troubles perceptivo‐moteurs chez l'enfant hemiplégique 18 enfants atteints d'hémiplégie ont été examinés en faisant appel à l'examen neurologique standard et à des sub‐tests spécialisés pour définir l'étendue de leurs troubles perceptivomoteurs. Une attention particulière a été accordée aux fonctions auditives du langage, aux praxies, aux performances visuo‐spatiales, au schéma corporel, à la défense tactile et à l'inattention. A l'exception de l'inattention, observée seulement dans l'hémipélgie gauche, le cǒté de l'hémiplégie ne s'est montré prédisposéà aucun trouble spécifique. La fréquence des problèmes de langage, en particulier dans les aspects auditifs du langage et dans les retards a été trouvée plus grande chez les enfants présentant une histoire de gaucherie familiale. Les résultats sont discutés à la lumière des théories de plasticité et d'organisation cérébrate. ZUSAMMENFASSUNG Perceptive Motordysfunktion bei Kindern mit Hemiplegie 18 Kinder mit Hemiplegie wurden anhand geläufiger neurologischer und anderer spezieller Tests untersucht, um das Ausmaß ihrer perceptiven Motordysfunktion festzustellen. Es wurde besonders auf die Funktion der auditiven Sprache, die Handlungsfähigkeit, das visuell räumliche Denken, das reale Körpergefühl, die tactile Abwehr und die Unaufmerksamkeit geachtet. Mit Ausnahme der Unaufmerksamkeit, die nur bei den Kindern mit linksseitiger Hemiplegie beobachtet wurde, wurde durch die Seite der Hemiplegie keine spezielle Dysfunktion praedisponiert. Kinder mit familiärer Linkshändigkeit hatten häufiger Sprachprobleme, insbesondere in Bezug auf auditive Sprache und Sprach verzögerung. Die Ergebnisse werden hinsichtlich der Theorien über Plastizität und cerebrale Organisation diskutiert. RESUMEN Disfunción perceptiva motriz en el niño con hemiplegia 18 niños con hemiplegia fueron examinados utilizando los substests neurológicos standard y otros substests especializados para definir la extensión de disfunción perceptiva motriz. Se puso particular atención en la función auditoria‐lenguaje, praxia, habilidades viso‐espaciales, esquema corporal, defensividad tactil e inatención. Con excepción de la inatención que fue vista solamente en aquellos casos con hemiplegia del lado izquierdo, el lado de la hemiplegia no se vio que predispusiera a ninguna disfunción específica. Se vió que los niños con una historia de zurderia familiar presentaban una incidencia mayor de problemas en el lenguaje, particularmente en el área del lenguaje auditivo y retraso en el habla. Se discuten los resultados a la luz de las teorías de la plasticidad de la organización cerebral.
