The Place of Surgery in the Treatment of Epilepsy in Childhood and Adolescence: A Preliminary Report in 13 Cases

SUMMARY Nine cases of childhood temporal lobe epilepsy which were resistant to drugs and were therefore treated by anterior temporal lobectomy, and four cases of severe epilepsy treated by hemispherectomy, are described. Clinical features and investigations are outlined. In general, there was a marked improvement in the patients' epilepsy and also some improvement in behaviour after surgery. The effect on their social adjustment and education is discussed. RÉSUMÉ La place de la chirurgie dans le traitement de l'épilepsie durant l'enfance et l'adolescence Neuf cas d'épilepsie temporale de l'enfance, résistants aux médications et de ce fait traité par lobectomie temporale antérieure ainsi que quatre cas d'épilepsie grave traités par hémisphérectomie sont décrits. Les symptômes cliniques et les données d'examen sont rapportés. Il y a eu en général amélioration marquée de l'épilepsie des sujets ainsi que de leur comportement après chirurgie. L'effet sur l'adaptation sociale et l'éducation est discuté. ZUSAMMENFASSUNG Die bedeutung der Chirurgie bei der Behandlung der Epilepsie im Kindes‐ und Erwachsenenalter Es werden neun Fälle mit kindlicher Temporallappenepilepsie beschrieben, die auf Medikamente nicht ansprachen und deshalb durch vordere Temporallappenentfernung behandelt wurden, und vier Fälle, die eine schwere Epilepsie hatten und durch Hemis‐phärektomie behandelt wurden. Die klinischen Merkmale und Untersuchungen werden dargestellt. Im allgemeinen war eine deutliche Besserung der Epilepsie und ebenfalls eine gewisse Besserung im Verhalten der Patienten nach dem chirurgischen Eingriff zu ver‐zeichnen. Es werden die Auswirkungen auf ihre soziale Eingliederung und Erziehung diskutiert. RESUMEN El lugar de la cirugía en el tratamiento de la epilepsia en la infancia y la adolescencia Se describen nueve casos de epilepsia del lóbulo temporal en la infancia resistentes a los fátrmacos y tratados con una lobectomía temporal anterior y 4 casos de epilepsia grave tratada con hemisferectomía. Se subrayan los caracteres clínicos y las investigaciones realizadas. En general hubo una mejoría marcada en la epilepsia del paciente y también cierta mejoría en el comportamiento después de la cirugía. Se discuten los efectos sobre su adaptación social y su educación.
