The Prevention of Cerebral Palsy

SUMMARY The provision of therapy and social support for spastic children has rightly been given priority for the past 25 years, but the author suggests that the prevention of cerebral palsy must now be the ultimate goal. Preventive measures based on knowledge already amassed are discussed and possible approaches to future research are surveyed. While the costs of such research and the implementation of preventive measures on a national scale may be high, there may well be an over‐all saving to the community if these costs are set against those incurred in the provision of special services for children with cerebral palsy and other manifestations of brain disorder. There would also be immense benefit in terms of human health and happiness. RESUME Prévention de l'infirmité motrice cérébrale Le traitement et l'assi stance sociale aux enfants IMC ont reçu une haute priorité durant les 25 dèrnieres années mais l'auteur suggère que la prévention de l'infirmité motrice cérébrale doit être maintenant le but ultime. Des mesures préventives basées sur les connaissances déjà acquises sont discutées et des voies d'approche pour la recherche future sont proposées. Quoique les couts de telles recherches et l'établissement de mesures préventives à l'échelle nationale peuvent être élevés, il pourrait se faire qu'il y ait au total un bénéfice pour la communauté si le coût est comparéà ce qui a été déjà dépensé pour l'établissement de services aux enfants IMC ou avec d'autres manifestations de lésions cérébrales. Le bénéfice serait de toute façon considérable en termes de bien être humain et de bonheur. ZUSAMMENFASSUNG Die Verhinderung der Cerebralen Lähmung Die Vorsorge in Therapie in sozialer Unterstiitzung für spastische Kinder ist in den letzten 25 Jahren zu Recht besonders gefördert worden, aber der Autor schlägt vor, daß jetzt die Verhinderung der cerebralen Lähmung das höchste Ziel sein müsse. Präventive Maßnahmen auf dem Boden des bereits angehäuften Wissens werden diskutiert und die Möglichkeiten einer zukünftigen Forschung aufgezählt. Während die Kosten für eine solche Forschung und die Verwirklichung der präventiven Maßnahmen auf einer nationalen Ebene hoch wären, könnte man insgesamt wohl für die Gemeinschaft sparen, wenn man diese Kosten mit denjenigen vergleicht, die bereits in der Pflege und den Spezialabteilungen für Kinder mit cerebraler Lähmung oder anderen Erkrankungen des Gehirns verbraucht werden. Auch in Hinsicht auf die menschliche Gesundheit und das Wohlbefinden würde daraus großer Gewinn erwachsen. RESUME N La prevención de la parälisis cerebral En los últimos veinticinco años se ha dado, con razón, prioridad a la terapéutica curativa y a la ayuda social de los niños espásticos, pero el autor sugiere que actualmente la finalidad última debe residir en la prevención de la parálisis cerebral. Se discuten medidas preventivas basadas en conocimientos ya acumulados y se revisan posibles enfoques de una futüra investigación. Mientras que los costos de tal investigatión y la implantatión de medidas preventivas a escala nacional pueden ser altos, ello puede constituir después de todo un ahorro para la comunidad si estos costos se comparan con los ya existentes en la provisión de los servicios especiales para niños con parálisis cerebral y otras manifestaciones de alteratión cerebral. Además ello daría un inmenso beneficio en el campo de la sanidad humana y del bienestar.
