SUMMARY Ricky is a boy with phenylketonuria associated with severe mental retardation who was kept at home for an unusually long time. His case thus provided an opportunity for studying the effects of home placement upon the child and his family. Despite moderate to severe mental retardation, the boy has maintained a higher degree of social maturity and adjustment despite what appears to be declining intellectual functioning. The cost of this advantage to the child has been a disturbed family setting requiring considerable counselling assistance. The parents differ considerably in their reactions and it is suggested that in such circumstances opportunities for parents to express their real feelings and motivations is necessary before their situation can be realistically appraised. Here placement of retarded children can be of significant value but the task of social casework falling to the medical practitioner may be considerable. Malgré une arriération mentale assez sévère et ce qui semble avoir été un déclin progressif du fonctionnement intellectuel, ce garçon a conservé un assez haut degré de maturité sociale et d'adaptation. Un milieu familial perturbé nécessitant une assistance psychologique considerable a été le prix de cet avantage pour l'enfant. Les parents diffèrent beaucoup dans leurs réactions et on pense que, dans ces circonstances, il faut donner aux parents l'opportunité d'exprimer leurs sentiments réels avant de faire le point de la situation de façon réaliste. Le placement à la maison des enfants arriérés peut avoir une valeur réelle, mais la tâche sociale individuelle qui incombe alors au médecin peut être considérable. El precio de esta ventaja para el niño ha sido un ambiente familiar perturbado, necesitando no poca ayuda bajo la forma de aconsejamientos. Las reacciones del padre y las de la madre son bastante diferentes y se considera que en tales circunstancias es necesario proveer ocasiones para que los padres expresen sus verdaderos sentimientos y motivaciones antes de que sea posible de valorar efectivamente su estado. La acomodación en casa de niños atrasados puede ser de valor significante pero la carga de trabajo social para el practicante médico es considerable.
