The nursing process is a very useful tool for involving and maintaining individualised support and care for people with mental handicaps. It also provides nurses with a methodology that can enhance their professional input into the multidisciplinary function. The article describes how the Nursing Process Monitoring Group in Frenchay Health Authority implemented the system in a fairly large unit, and provides examples of the documentation used for the benefit of others who may wish to follow suit.
