Abstract Clinical guidelines are prominent in the medical practice. Many practice guidelines have recently been published for the treatment of plaque psoriasis. Although they provide useful information on how the different treatments should be applied, unfortunately, most of them do not give instructions on the sequence in which these treatments should be prescribed. In the light of recent publications proposing algorithms and the clinician's paradigm in the treatment of psoriasis, three key questions are discussed in this paper: How do we evaluate the severity of psoriasis? Which treatment should be given in order of preference? And how do we make the therapeutic decision? It is generally agreed that each patient requires an individualized treatment plan and that the decision to undertake systemic treatment and the choice of the specific treatment plan must ultimately and mutually be made by both the patient and the physician.
