Fixed drug eruptions (FDE) are common but their pathogenesis and site specificity is poorly understood. We report the occurrence of FDE related to trimethoprim‐sulfamethoxazole in four siblings. A new hypothesis of familial occurrence of FDE and its pathogenesis is proposed.
