A rare case of a cerebral venous and sinus thrombosis with a four year course of recurring focal neurological deficits as a paraneoplastic remote effect of a later diagnosed highly malignant lymphoma is presented. Initially heparin‐therapy and anticoagulation led to good clinical recovery. The subsequent failure of this treatment and a surprisingly good recovery immediately after chemotherapy is discussed.
