Difficulties in access to health records for research, including ‘catch 22’ situations where researchers need consent from patients to access the medical records to identify those patients. The need for large or national studies to be reviewed by many ethical committees, involving much time and cost. Opportunities for development would arise from improvements in health information and in ethical review procedures, and from expansion of training and career positions. Better integration of health research with health care delivery is needed. Evidence-based health care is poorly developed and there is little commitment of health care funders to research, development or evaluation. Provision of 1% of the health budget for research and development, less than that in the UK, would dramatically improve the situation.
