Advice given to patients about the use of alcohol is often confusing, and different sources may give conflicting advice. For example, Tattersall’ recommended that the carbohydrate content of alcoholic drinks should be included in the usual daily allowance of carbohydrate whereas MannZ said it should not. Some doctors advise the use of low carbohydrate lagers and beer^,^ and others say they should be avoided.* The only advice so far given by the Nutrition Subcommittee of the British Diabetic Association4 is that patients should ‘be cautioned against excessive alcohol intake and against drinking on an empty stomach’. This paper has two aims: to provide a summary of the known effects of alcohol on metabolic homeostasis in the diabetic patient and, in the light of this knowledge, to suggest what practical advice should be given to patients.
