Summary A group of 47 patients with hypertension were followed for up to 16 years after unilateral nephrectomy. The principal indication for the nephrectomy was hypertension in the presence of unilateral renal disease. Methods of investigation and the pathological lesions are described. 60% of those patients followed for 7 years have maintained normal blood pressures without drug therapy. All of the patients with normal heart size and no hypertensive changes in the remaining kidney, remained normotensive for 5 years; for these reasons the prognosis was better in those under 40 years of age. Where nephrectomy failed to lower the blood pressure the requirements for hypotensivetherapy were considerably reduced.
