SUMMARY Two patients with idiopathic priapism are described. Some aspects of the use of surgical by‐pass are discussed. The obstruction to venous return from the corpora cavernosa in priapism, its treatment by venous by‐pass, and the ultimate development of a new venous drainage from the corpora, are demonstrated by cavernosography. Even when priapism has existed for a number of days, surgical venous by‐pass will induce detumescence and may preserve potency.
