SUMMARY Eighteen patients with xanthogranulomatous pyelonephritis were treated at the Mayo Clinic during the past 53 years. The variable and often deceptive rœntgenographic appearances, often matched by diagnostic inaccuracies confounded by a hepatic dysfunction syndrome akin to that produced by renal cell carcinoma, are often associated with equally confusing and diverse manifestations of the disease closely mimicking those of hypernephroma, perinephric abscess, pyelonephritis with stone, renal carbuncle, and tuberculosis of the kidney. The common denominator is renal sepsis with localised or widespread necrosis and abscess formation. Three different stages of the disease and suitable methods of treatment for each are described. The disease follows a benign course. There are no recurrences and the results of treatment are most gratifying.
