SUMMARY Five cases of pyocystis syndrome are presented where conservative treatment failed. Two cases needed simple cystectomy and 2 were cured with the creation of a urethro‐vaginal fistula as suggested by Spence. One bladder was sclerosed with a chemical irritant leading to scarring and cessation of discharge. The aetiology of the bladder secretion is discussed and the relevant literature is reviewed.
